Care Choices is a wholly owned subsidiary of Trinity Health Plans, a management services company that is a part of Trinity Health, one of the largest health insurance companies in the US. The parent company, formed by the merger of the Congregation of the Sisters of the Holy Cross and Mercy Health Services, traces to 1841.
