Community Choice Michigan
Community Choice is a non-profit Medicaid managed health care plan organized in 1995 to assist Michigan's Medicaid patients. The plan is sponsored by a network of 16 Federally Qualified Health Centers, all non-profit community health clinics that supply health care to medically underserved individuals. These community health clinics have been providing primary medical, dental, mental, and preventive health care to Michigan citizens for over thirty years. The CareSource Management Group provides a full range of administrative services for this plan, including a 24-hour nurse hotline. Community Choice is used by 47,000 individuals in 36 Michigan counties all over the state. The plan follows the HMO model of health insurance, with an emphasis on managed care by a primary physician.
Community Choice Health Insurance Products
Community Choice Michigan works diligently to keep their members healthy. The plan covers all services that Medicaid Michigan requires. Other features include the following:
- Transportation is provided to doctor visits
- Stop-smoking assistance through the Call It Quits program
- Babies First program provides discount card incentives for program participation
- No co-payments
- Newsletters, brochures and other educational materials
- A service center that also offers TTY for the hearing impaired
- Services provided by a specialist are available. Some of these services require a referral from your primary care doctor, while others do not need a referral.
- Prescription drug coverage
- Vision and mental health services are available
- A case manager to help you manage your health issues, including education and help for chronic conditions such as asthma, diabetes, obesity, and heart failure
