Great Lakes Health Plan, Inc. is an AmeriChoice health plan, designed to serve Medicaid, Child Health Insurance Programs (CHIP), and Medicare beneficiaries. Today, Great Lakes Health Plan is the second largest Medicaid health plan in Michigan, with over 140,000 members in 19 Michigan counties. Members must be enrolled in Michigan Medicaid to receive services. The areas served are in southern and eastern Michigan.
Great Lakes Health Plan is structured as an HMO. Members choose a primary care doctor, who manages the patient's health care. The emphasis is on preventive medicine and staying well.
