Physician's Health Plan - Mid MI
The Physicians Health Plan of Mid-Michigan is a non-profit health maintenance organization that was formed to provide quality health care to patients in their area of Michigan. Formed in 1980, the group is a part of the Sparrow Health System. This health plan is available to employers and their employees and to Medicaid recipients within the central Michigan counties served who have chosen this plan.
Physicians Health Plan of Mid-Michigan's Health Insurance Products
Physicians Health Plan of Mid-Michigan offers the following affordable health insurance plans for Michigan families. HMO and high deductible health plans (HDHP) are flexible and offer a wide range of deductibles and co-pays. Some plans allow members to choose their own physician. Others allow a combination with a health savings account (HSA). Prescription and vision riders are available. Michigan Medicaid benefits are standard.
- HMO plans that focus on a strong doctor-patient relationship with a primary care physician. Preventive care programs and incentives help keep patients well. Members pay a pre-determined, pre-paid amount per member in order to receive basic health care. Co-pays vary but are small, i.e., $10 per visit, with a 10 percent coinsurance.
- POS plans to give the insured the option to pay at point of service for care outside the plan network. An HMO hybrid plan, the POS allows reimbursement of some costs for out-of-network care. Members do not have to choose how they will receive their care until they need that care.
- PPO plans with the flexibility to choose deductibles and coinsurance. These plans allow greater freedom to select providers. Providers within the network charge discounted rates for their services.
