Physician's Health Plan - Southwest MI
The Physicians Health Plan of Southwest Michigan was purchased in September of 2006 by Great Lakes Health Plan, which is affiliated with United Healthcare. The group is focused on providing quality health care service to the citizens of selected counties in southwestern Michigan. This health plan is available to employers and their employees and to Medicaid recipients within the Michigan counties served who have chosen this plan. Today, Great Lakes Health Plan is the second largest Medicaid health plan in Michigan, with over 140,000 members in numerous Michigan counties. Members must be enrolled in Michigan Medicaid to receive services. Great Lakes Health Plan is structured as an HMO. Members choose a primary care doctor, who manages the patient's health care. The emphasis is on preventive medicine and staying well.
